The 2008 convention registration form follows.

The fee is $200 for the delegate and $125 for their spouse and includes admittance to all events: the Friday meeting and ice cream social; the welcome reception and hors d'oeuvres on Friday evening; the Saturday a.m. breakfast; Saturday evening banquet; Sunday breakfast; and coffee breaks.
